(4) The dollar value of the awards shown represents the grant date fair value of the award computed in
accordance with ASC 718. See Note 8 in the Notes to Consolidated Financial Statements included in our
2013 Annual Report on Form 10-K for more information about the assumptions used to determine these
amounts.
(5) Represents an LTI PSU award granted to the named executive officer under our 2004 Performance Incentive
Plan for the performance period covering fiscal years 2013 and 2014. The award will be payable in shares of
our common stock at the end of the performance period based on our achievement of specified operating
income and revenue goals that correspond to specific payment percentages ranging between 0% and 200% of
the target number of units subject to the award.
